Yellow Fever Permit

The Animal Disease Prevention Division issues permits for medical doctors who provide Yellow Fever Vaccinations.  To obtain a permit for the vaccination, providers must store the vaccine at between two-eight degrees Celsius or thirty six-forty six degree Fahrenheit. Providers must maintain a 24 hour record showing that the temperature has been maintained in the acceptable range.  Permits expire annually on June 30, and a yearly inspection of records is required.

Call (202) 535-2323 for an application or inspection.  There is no charge for this permit.
